Incidents and investigations

Incidents reported in minutes, investigated properly and closed with evidence.

Anyone can report an incident, near miss or hazard from a phone or the web, and the form fills itself from their description. Administrators are alerted straight away, notifiable and serious incidents are flagged, and investigations follow a guided method through to key learnings and close.

How it works

From first report to verified close out.

1

Report

Describe what happened in plain words. The form picks up the people, place, injury and kind of incident, with a body map, photos and first aid given.

2

Classify and notify

The incident is reviewed and given a level. Notifiable incidents under the WHS Act and serious incidents in early learning are flagged with the time limits that apply.

3

Investigate

Basic, systemic or root cause analysis with a timeline, evidence, five whys and contributing factors, signed off by the investigation team.

4

Learn and close

Corrective actions are assigned and tracked to close, and the learnings become a toolbox presentation for the people who need them.

What regulators, auditors and clients expect.

What the law or standard asks of you, in plain words, and what MiSAFE QHSE does to help you meet it.

Tell the regulator about a death, serious injury or illness, or a dangerous incident Each report is checked against the notifiable list as it comes in, and anything notifiable is flagged to an administrator straight away.
WHS Act s35 to s38
Leave the scene untouched until the regulator says otherwise The report reminds the person in charge to secure the site, and photos taken on the phone are kept with the record.
WHS Act s39
Keep a record of every incident, injury, trauma and illness in early learning Early learning incidents are recorded on the same form, with the details the Regulations ask for and a body map of any injury.
National Regulations r87
Tell the regulatory authority about a serious incident in early learning within 24 hours Serious early learning incidents are flagged with the 24 hour deadline so the notification is not missed.
National Regulations r176
Investigate, fix the cause and check the fix worked Investigations follow a guided method, every corrective action has an owner and a due date, and someone else confirms it worked before it closes.
